Important Filing Changes:

From October 1, 2009, the Department of Buildings is only accepting the 10/09 version of the TR-1, TR-2 and TR-3 forms.

The new TR-1 has the three changes, outlined below.
1. New item for Alternative Materials
2. Changes to Concrete Design Mix & Concrete Test Cylinders Items
3. Final Inspection Report

1. New item for Alternative Materials
Special inspection responsibilities for “Alternative Materials” is documented on the TR1. A series of OTCR Building Bulletins is issued by Dept of Buildings to define what materials are allowed.

2. Changes to Concrete Design Mix & Concrete Test Cylinders Items
Concrete Design Mix and Concrete Test Cylinders items are moved to the bottom of section 3 of TR-1.

  • To identify responsibility and certify completion, a TR-3 must completed and submitted prior to permit for Concrete Design Mix.
  • The TR2 must be submitted to identify responsibility prior to permit and again to certify completion prior to signoff for Concrete Test Cylinders.
  • 3. Final Inspection Report
    To better monitor the quality and standard of work performed on projects not needing a new or revised Certificate of Occupancy, the Department requires a final inspection report be submitted within 30 days of the final inspection.

    In these reports, final inspection applicants must attest that the work is compliant with:

  • Approved Plans
  • Building Code
  • Any other Applicable Laws and Rules
  • The New TR-2
    Prior to permit, the Department requires a Concrete Test Cylinder Technical Report to identify responsibilities for inspections. This report is also required prior to sign off to certify completion of concrete pouring, sampling and compression test cylinders.
    A Department-licensed Concrete Testing Lab must submit the TR2, and the lab must attach the trial mixture reports and/or the field experiment results.

    The new TR2 includes the following additional and required sections:

  • Design Applicant Statement and signature in Identification of Responsibilities and Certification of Completion (required prior to permit and prior to sign-off)
  • Building Owner Statement and signature (required prior to permit)
  • Concrete Producer’s Statement and signature (required prior to sign-off)
  • The New TR3
    Prior to permit on all jobs in which concrete will be used, the Department requires a Concrete Design Mix Technical Report.

    The new TR3 includes the following additional required sections:

  • Design Applicant’s statement and signature
  • Building Owner’s statement and signature
  • Concrete Producer’s statement and signature

  • When more than one concrete testing report is required, additional TR2 and TR3 forms must be submitted.
